A succinct answer!
Recreational - only if specifically indicated by a clincial suspicion of disease to the lungs/chest.
Commercial - every 5 years unless some specific reason for more frequently.

Recreationally I only perform a CXR if it is clinically indicated and it is an optional item on the dive medical.
Commercially (depending on the juristiction - eg AS 2299 or DMAC-EDTC etc) it can be mandatory or at the physicians discretion.
